Aging Life Care Professional Services

You shouldn't have to figure this out alone.

Caring for an aging parent is one of the most loving things a family can do — and one of the most exhausting. The doctor's appointments, the medication changes, the housing decisions, the family disagreements, the sleepless nights wondering if you're making the right choices. It's a lot to carry.

That's exactly what we're here for.

As Certified Aging Life Care Professionals serving Metro Atlanta and across Georgia, we step in as your guide, your advocate, and your expert partner. We assess the full picture, build a personalized care plan, coordinate every piece of the care puzzle, and stay by your side through every transition — so you can stop managing and start being present with the person you love.

What is an Aging Life Care Professional?

An Aging Life Care Professional (ALCP) — sometimes still called a Geriatric Care Manager — is a trained specialist in the health, social, financial, and emotional needs of older adults. We are advocates and coordinators, not caregivers who provide hands-on personal care. Think of us as the quarterback of your loved one's care team: we make sure everyone is communicating, nothing falls through the cracks, and every decision is made with the full picture in view.

How We Help

Geriatric care management is not a one-size-fits-all service. Every family we work with starts in a different place — some are managing a new diagnosis, some are navigating a hospital discharge, some are long-distance families who just need a trusted local presence. We meet you exactly where you are.

Comprehensive Assessment

We begin with a thorough assessment of your loved one's physical health, cognitive function, emotional wellbeing, living environment, and social supports. This gives us — and you — a clear, honest picture of where things stand and what's needed.

Personalized Care Planning

Every care plan we develop is built around the individual — their values, their preferences, their medical needs, and their family dynamics. We don't hand you a generic checklist. We hand you a roadmap.

Care Coordination

We work directly with physicians, specialists, therapists, home care agencies, and senior living communities to ensure everyone is on the same page. We attend appointments, ask the right questions, and translate medical information into language families can actually use.

Housing & Placement

Is it time to consider assisted living? Memory care? In-home support? We help families evaluate all the options — not based on what's convenient, but based on what's truly right for your loved one. We know the communities in Metro Atlanta and can guide you toward the right fit.

Crisis Intervention

When emergencies happen — a hospitalization, a fall, a sudden decline — we respond. We help manage transitions, coordinate discharge planning, and ensure nothing gets missed in the chaos of a crisis.

Family Support & Mediation

Care decisions can be some of the most emotionally charged conversations a family has. We help facilitate those conversations — with compassion, honesty, and the clinical grounding to cut through the noise.

Ongoing Monitoring

For families who want continued peace of mind, we provide regular check-ins, reassessments, and proactive updates. Things change. We change the plan with them.
