An Aging Lifecare Professional (previously Geriatric Care Manager) is a specialized approach to addressing the unique needs of older adults, enhancing their overall quality of life. Care management achieves this through comprehensive support including care assessment & planning, care coordination, resource recommendations, health monitoring, crisis intervention, family support & education, and advocacy.
