When a stroke happens, your world turns upside down in an instant. Emergency hospitals like Grady Trauma or Emory save lives — and for that, families are grateful every single day. But once the immediate crisis passes, a new and much quieter problem begins: the hospital moves to discharge your loved one home before your family is ready, and before the plan is actually safe.
This is one of the most stressful moments in a family's caregiving journey, and it happens fast. Understanding your options now — before a case manager hands you a discharge folder — is the best way to protect your loved one's recovery and your family's future.
Stroke Recovery Depends on the Level of Brain Injury
Not every stroke survivor needs the same next step. The right setting after hospitalization comes down to how much therapy your loved one can tolerate and how much support they need to stay safe.
Acute Rehab (Inpatient Rehab)
Stroke survivors who can participate in three hours of therapy a day — combining physical, occupational, and speech therapy — belong in a dedicated acute inpatient rehab facility. This intensive setting is designed to rebuild function as quickly and safely as possible during the critical early recovery window. In Metro Atlanta, families often turn to facilities such as Shepherd Center Atlanta, Emory Rehabilitation Hospital, and Encompass Health.
Subacute Rehab (Nursing Rehab)
Not every stroke survivor is strong enough for that pace. Patients who are too weak, too fatigued, or too medically fragile for three hours of daily therapy typically do better in a subacute rehab center, where therapy is gentler and paced to the patient's tolerance. In the Atlanta area, centers such as Salude offer this slower, more supportive level of rehab.

The Big Shock: What Home Care Really Costs in Atlanta
When hospital staff say, "Just take them home and hire some help," most families have no idea what that actually costs.
In Metro Atlanta, quality home care agencies — such as CaraVita — typically charge between $32 and $38 per hour. If your loved one needs around-the-clock care to stay safe at home, the math adds up quickly:
Daily cost: roughly $670–$910/day
Monthly cost: $25,000 to $30,000 or more, every single month
Medicare does not cover 24/7 caregiving at home. Families paying out of pocket can burn through a lifetime of savings in a matter of months — often without realizing how fast the clock is running until the bills arrive.
Why Assisted Living Might Say "No"
Many families assume that if home care isn't realistic, assisted living is the fallback. But Georgia law sets strict limits on who an assisted living community can accept.
Under Georgia regulations, assisted living residents generally must be able to "self-preserve" — meaning they need to be able to help save themselves in an emergency, such as a fire, without staff physically carrying them out.
An assisted living facility usually cannot accept a stroke survivor who requires:
A Hoyer lift. Standard assisted living staff are not trained or staffed to operate mechanical lifts for transfers.
Two-person transfers. If it takes two staff members to safely move your loved one, most assisted living communities cannot meet that need.
Neurogenic bowel or bladder management. Nerve damage from a stroke often requires skilled nursing-level care that standard assisted living is not licensed or equipped to provide.
Families frequently learn this the hard way — after a tour, a deposit, or even a move-in — rather than before. Knowing these criteria in advance changes how you plan.
Nursing Home Care & the Spousal Financial Trap
When a stroke survivor needs care around the clock, a long-term Skilled Nursing Facility (SNF) often becomes the only realistic option. But that level of care isn't cheap: expect $8,800 to $11,000 a month out of pocket in the Atlanta area.
For married couples, this creates a serious and often overlooked danger. If the healthy spouse continues paying full nursing home costs out of the couple's shared assets, they can be left with little or nothing to live on themselves — a risk sometimes called spousal impoverishment. Without proactive planning, a stroke can financially devastate not just the patient, but the spouse who is still at home trying to hold the family together.
